FVM v2.1 — Forensic Vertical Matrix

Determine whether a distributed condition is actually systemic — and whether the organization can govern it.

FVM is a bounded systemic-governability phase used when the remaining question is no longer merely evidentiary, but depends materially on cross-boundary relationships, recurrence, propagation, interdependence, or governance-response capacity.

It maps conditions across domains and lifecycle stages, tests whether relationships are static, distributed, or propagating, and determines what can responsibly be concluded about governability.

FVM does not treat the existence of a matrix as proof that a system is behaving dynamically.

Dynamic, systemic, attribution, capital, and governability conclusions must be earned separately from the evidence record.

A map is not a model.

A relationship is not propagation.

Cross-silo presence is not systemicity.

Materiality is not systemicity.

Unknown is not failure.

Continuity & Re-Examination

A deeper phase must still be able to correct the phase before it.

FVM inherits prior findings, but it does not lock them.

An observed condition may carry forward where its evidence provenance remains valid.

A prior causal level may be strengthened, weakened, reframed, or left unresolved.

A prior containment conclusion is treated as a hypothesis about local governability and is re-tested where cross-boundary behavior matters.

Evidence classes and conflicts remain bounded by the evidence actually available.

A prior decision to enter FVM establishes why deeper systemic examination is warranted. It does not establish that the condition is systemic.

An inherited concern is not a terminal fact merely because it triggered deeper examination.

A deeper phase should retain the authority to correct the phase before it.

Scope, Ontology & Boundary

The system boundary must be represented honestly.

A matrix is useful only if it can represent the commissioned question without distorting it.

FVM therefore does not force every cause, dependency, event, or organizational condition into a domain × lifecycle cell merely because the matrix uses that structure.

Where a material condition cuts across the grid, it remains visible separately.

External / Common Cause Market shock, regulation, macro event.
Infrastructure / Platform Migration, shared service, vendor dependency.
Policy / Incentive Compensation change, policy redesign, operating incentives.
Measurement / Definition Metric definition, reporting logic, or data-model change.
Leadership / Organizational Reorganization, departure, or decision-right redistribution.

The question is not whether every fact can be placed neatly into the matrix.

The question is whether the matrix and its surrounding evidence record are capable of representing the system honestly enough for the decision being examined.

A useful framework should represent the problem — not force the problem to resemble the framework.
What does not fit the grid still belongs in the evidence record.
Dynamic-Evidence Rule

Dynamic claims require dynamic evidence.

FVM inherits the Obraval evidence architecture, but systemic examination introduces an additional evidentiary burden.

Claims about:

  • recurrence;
  • velocity;
  • compounding;
  • feedback;
  • propagation;
  • or governance-response capacity

require evidence appropriate to dynamic behavior.

That may include longitudinal records, event sequencing, repeated observations, or other relationship-specific evidence capable of supporting the claim being made.

A static snapshot may show that two conditions exist at the same time.

It does not, by itself, establish that one condition is moving into another, recurring across boundaries, feeding back on itself, or outrunning the organization’s governance capacity.

Static evidence may reveal a relationship. It cannot, by itself, prove a dynamic system.
Matrix Architecture

The matrix records conditions — it does not prove system behavior.

FVM uses a matrix to make material governance conditions visible across domains and lifecycle stages.

Columns represent domains or silos.

Rows represent lifecycle stages.

Cells represent bounded governance-condition observations.

Governed

Evidence supports that the relevant decision or control responsibility is sufficiently owned, operated, monitored, and decision-useful within the stated boundary.

B

Bounded Fracture

A governance-relevant condition exists, but the evidence supports a local boundary and no material unresolved propagation pathway.

D

Distributed Condition

A related condition appears across more than one cell, domain, or stage, but the direction or systemic mechanism has not been established.

Supported Systemic Propagation

Evidence supports a recurring or dynamic propagation mechanism across defined boundaries, with directionality or feedback appropriately tested.

U

Unresolved / Insufficient Evidence

Evidence is too incomplete, conflicting, weak, or non-representative to support a stronger state.

A distributed condition does not equal systemic propagation.
A governed cell does not prove the wider system is governed.
Governed-State Floor

A documented control is not the same as a governed condition.

FVM does not treat the existence of governance architecture as proof that governance is operating effectively.

A named owner is not enough.
A written policy is not enough.
A documented control is not enough.
Formal authority is not enough.
An org-chart placement is not enough.

A governed classification requires evidence of operating reality.

It also requires evidence that the control is directed at the material condition it is supposed to detect, constrain, or govern in the current operating environment.

A control may operate exactly as designed and still fail to establish governability if it is aimed at an obsolete, incomplete, or materially different risk surface.

Control existence is not control effectiveness.
A functioning control aimed at the wrong problem does not establish governability.
Relationship & Dependency Mapping

Relationships are examined separately from cell status.

A cell tells leadership something about the condition present at a location in the matrix.

It does not, by itself, explain how that condition relates to another location.

FVM therefore separates cell state from relationship state.

This matters because a condition can appear in several places for very different reasons.

Several functions may depend on the same vendor.

Several business units may be affected by the same policy change.

Several systems may reflect the same measurement change.

Several areas may degrade at the same time without one transmitting the condition to another.

Presence across multiple boundaries is not proof of transmission across those boundaries.
Directionality Test

Directionality must be earned.

Where a relationship appears to exist, FVM tests the direction of that relationship rather than assuming it.

A → B may be supported.
B → A may remain plausible.
C may explain both A and B.
A ⇄ B feedback may be present.
Or the apparent relationship may simply reflect co-occurrence.

Where direction cannot be responsibly established, the relationship remains unresolved.

A relationship can exist without leadership yet knowing which way the influence travels.

Temporal sequence may make one direction more plausible.

It does not, by itself, prove the mechanism.

Propagation Evidence Ladder

Propagation has an evidence ladder.

FVM does not use the word systemic as a general description of complexity.

Propagation strength is classified.

P0

No Supported Relationship

Conditions may co-occur, but the evidence does not support a relationship.

P1

Possible Pathway

Architecture or sequence makes transfer plausible, but transfer has not been observed.

P2

Observed Transfer

Evidence shows that a condition or event crossed a defined boundary in at least one examined instance.

P3

Recurring Propagation

Repeated or longitudinal evidence supports recurrence of the transfer mechanism across the defined boundary.

P4

Systemic Propagation

Recurring propagation materially spans multiple domains or lifecycle stages, or creates feedback or compounding behavior that cannot responsibly be treated as isolated transfer.

PU

Dynamic Behavior Unresolved

Evidence cannot responsibly distinguish among transfer, common cause, correlation, reverse direction, or another explanation.

Multiple affected cells do not prove propagation.

A shared dependency, market condition, system migration, policy change, or measurement change can create correlated degradation without one cell transmitting a condition to another.

Propagation must be demonstrated, not inferred from pattern density.

Competing System Explanations

Competing system explanations remain active.

A coherent systemic explanation can still be wrong.

FVM therefore requires material propagation and systemic conclusions to be tested against credible alternatives.

  • common cause;
  • reverse direction;
  • independent local failure;
  • measurement artifact;
  • feedback;
  • or non-propagating dependency.

A token alternative does not satisfy the requirement.

The alternative must be one a reasonable, informed reviewer could regard as plausible given the facts, chronology, architecture, external context, and known confounders.

Systemic coherence is not the same as systemic proof.
Competing Explanation Rule

When the evidence cannot distinguish the explanations, the methodology should not choose for it.

Where a proposed propagation hypothesis and a credible competing explanation are supported at materially equivalent evidentiary strength, propagation is not established.

The relationship remains:

PU — Dynamic Behavior Unresolved

until evidence materially differentiates the explanations.

Narrative coherence does not break the tie.
Executive preference does not break the tie.
Operator preference does not break the tie.
Perceived plausibility does not break the tie.
When the evidence cannot distinguish propagation from its strongest alternative, the methodology should not choose for it.
Condition Lineage

One underlying condition may appear many times without becoming many root causes.

A systemic map can make one underlying condition look like several independent problems.

FVM controls for that through condition lineage.

The methodology distinguishes among:

  • root conditions;
  • manifestations;
  • downstream effects;
  • feedback effects;
  • and unknown relationship states.

It also links those manifestations to shared exposure identities so the same underlying condition is not counted repeatedly merely because it appears in multiple cells, stages, or causal chains.

Distributed visibility should not become duplicated causality or duplicated exposure.
Canonical Exposure Reconciliation

Exposure must be reconciled before it is aggregated.

FVM requires financial and operational exposure to reconcile to canonical exposure identities before board-level aggregation.

A new exposure cannot be created merely because the same condition appears in another part of the matrix.

Where only part of an exposure is additive, the distinct non-overlapping component must be specifically evidenced.

The same underlying exposure should not become larger simply because the matrix shows it in more places.

Repetition across the matrix does not create additional financial magnitude.
Exposure must be reconciled before it is aggregated.
Attribution Strength

Attribution remains evidence-bounded and revisable.

FVM does not use an attribution lock.

Attribution remains bounded by the evidence and capable of reopening when the record changes.

Observed Ownership

A role, function, system, or control is directly associated with the observed condition.

A2

Supported Responsibility

Evidence supports that the party or system materially contributed to or governed the relevant decision pathway.

A3

Shared / Distributed Attribution

Multiple parties, systems, or decisions materially contribute, and no single owner explains the condition.

A4

Bounded Attribution

Evidence supports attribution within a defined scope while material alternatives and falsifiers remain preserved.

AU

Attribution Unresolved

The evidence does not responsibly support a stronger assignment.

Every material attribution must also state what evidence would weaken, redistribute, or reopen it.

Attribution should remain revisable when the evidence is revisable.

Later evidence may change attribution without implying that the earlier record was dishonest.

The relevant question is whether the earlier conclusion was defensible on the evidence available at the time.

Governability Examination

Governability is not inferred from matrix density.

FVM does not determine governability by looking at how many cells appear problematic.

It examines whether the organization can actually operate across the relevant boundary.

Visibility Can leadership see the materially relevant condition, dependencies, and uncertainty?
Ownership Is accountable ownership identifiable and accepted?
Authority Can the accountable role act across the necessary boundary?
Capability Are the required skills, resources, and systems available?
Coordination Can affected functions integrate information and action across silos?
Control Effectiveness Is there evidence controls operate as intended under relevant conditions?
Monitoring Can recurrence or degradation be detected in decision-relevant time?
Escalation Can material conditions reach appropriate authority before exposure materially changes?
Adaptation Can governance response adjust when assumptions or conditions change?
Reversibility Can the condition be reduced, contained, reversed, or redesigned within a defined horizon?

Documentary ownership, formal authority, stated capability, policy existence, or organizational placement do not establish governability by themselves.

The conclusion requires demonstrated operating reality.

Governability is demonstrated through operating capacity, not visual pattern density.

Governance Response Capacity

Governance-response capacity must be evidenced on both sides.

Claims about whether governance can keep pace with a changing condition require evidence about both the condition and the organization’s response capacity.

Detection Latency How quickly can the condition become visible?
Escalation Latency How quickly can the issue reach appropriate authority?
Decision-Cycle Time How long does governed decision-making require?
Containment / Response Time How quickly can the organization act within the relevant boundary?
Monitoring Cadence How frequently is the condition observed?
Recurrence Interval How frequently does the condition reappear?
Remediation Throughput / Backlog What is the demonstrated response capacity relative to demand?

FVM does not claim that exposure is compounding faster than governance can absorb unless both sides of that comparison are actually supported.

If one side remains inadequately evidenced, the comparison remains unresolved.

A comparison is only as strong as the evidence on both sides of it.
Reversibility Classification

Difficult does not mean irreversible.

The language of irreversibility carries substantial executive weight.

FVM therefore distinguishes several materially different states.

R1

Readily Reversible

Evidence supports reversal or containment through ordinary governance action within a decision-relevant horizon.

R2

Reversible with Material Effort

Reversal is feasible but requires meaningful time, cost, coordination, or redesign.

R3

Slow / High-Cost Reversal

Reversal remains feasible, but implementation horizon or cost materially affects governance choices.

R4

Currently Uncontained

The condition is active or recurring, but current lack of containment does not establish irreversibility.

RU

Reversibility Unresolved

Evidence cannot responsibly establish whether or how the condition can be reversed.

R5

Structurally Non-Reversible within Defined Scope

Used only where evidence supports that the condition cannot be reversed within the stated scope and horizon and material alternatives have been tested.

Difficult, costly, slow, distributed, or currently uncontained does not equal irreversible.

Capital, Operational & Decision Exposure

Matrix severity does not create financial magnitude.

A serious systemic condition may have material economic implications.

But the matrix itself does not create the amount.

Observed Exposure Supported directly by evidence within the defined scope.
Modeled Exposure Estimated under stated assumptions and sensitivity.
Counterfactual Exposure Modeled against an evidence-supported alternative scenario.
Opportunity Exposure Potential foregone value rather than established loss.
Unresolved Exposure Magnitude cannot responsibly be determined from the available evidence.

Exposure must be separately observed or modeled.

Non-additive manifestations cannot be summed simply because they appear in several cells or relationship chains.

A wide numerical range that crosses materially different decision thresholds may still be too uncertain to support the decision.

Severity is not a financial model.
A serious condition can remain financially unresolved.
Board-Readable Matrix Brief

The board-readable matrix brief cannot strengthen the record.

Board communication may require complex systemic findings to be compressed.

FVM permits simplification of presentation.

It does not permit simplification of evidentiary limits.

The board-readable matrix brief may not:

  • remove a U state;
  • convert an unresolved relationship into propagation;
  • collapse exposure labels;
  • omit a material alternative explanation;
  • or soften a material scope or evidence limitation.

Cross-cutting conditions that could materially explain the distributed pattern, alter propagation strength, or affect the disposition must also remain visible.

Board readability may simplify the presentation. It may not simplify away the uncertainty.
Priority Relationship Chains

A relationship chain is only as strong as its weakest material link.

Systemic explanations often become persuasive because the overall chain appears coherent.

FVM does not permit visual or narrative coherence to substitute for evidence at each material connection.

A relationship chain may include several individually plausible links.

But if a load-bearing link remains unresolved, the chain cannot be presented as established systemic propagation.

A coherent story is not stronger than its weakest evidentiary link.

High-Authority Finding Challenge Gate

The stronger the conclusion, the stronger the challenge it should survive.

FVM requires independent challenge for its highest-authority findings.

That includes proposed:

  • P3 recurring propagation;
  • P4 systemic propagation;
  • S-state classification;
  • and redesign-level terminal conclusions.

The challenger must sit outside the primary evidence owner or hypothesis author.

The challenge must test:

  • the propagation mechanism;
  • the strongest material alternative;
  • the evidence-class fit;
  • and whether the proposed conclusion exceeds the weakest material link.

A superficial acknowledgment is not enough.

The stronger the conclusion, the stronger the challenge it should survive.
Governability Sufficiency Determination

Governability sufficiency is tested before terminal disposition.

FVM does not reach a terminal position simply because the matrix has been populated.

Before disposition, the methodology examines whether there is sufficient:

Matrix Adequacy Material cells and cross-cutting conditions are represented without forcing unknowns.
Evidence Adequacy Load-bearing systemic conclusions rely on evidence appropriate to their strength.
Relationship Adequacy Directionality, common cause, reverse direction, and dependencies are sufficiently examined.
Propagation Adequacy Propagation language does not exceed the supported P-level.
Lineage Adequacy Conditions and exposures are traced without double counting or false root-cause multiplication.
Attribution Adequacy Attribution strength matches the evidence and remains reopenable.
Governability Adequacy Operating capacity is sufficiently visible across the relevant boundary.
Capacity Adequacy Claims comparing exposure dynamics and governance capacity are supported on both sides.
Decision-Boundary Clarity Leadership can state what FVM supports, leaves unresolved, and cannot conclude.
Challenge Adequacy Material systemic conclusions received substantive independent challenge.
Competing-Explanation Reconciliation Material alternatives are reconciled or the finding remains unresolved.
Exposure Reconciliation Aggregate exposure is reconciled without duplication.
Terminal does not mean certain. It means the methodology has reached its legitimate decision boundary.
Terminal Dispositions

Five bounded terminal dispositions.

Disposition A

Governability Sufficiently Established

Evidence supports that the relevant system boundary is governable under current or explicitly bounded governance conditions. No further diagnostic phase is required on the current record.

Disposition B

Systemically Distributed but Governable

Material conditions span multiple boundaries, but governance remains viable through existing or bounded redesign capacity. Distribution does not equal ungovernability.

Disposition C

Dynamic / Systemic Evidence Incomplete

Material system behavior remains unresolved because relationship, propagation, governance-capacity, or reversibility evidence is insufficient. The uncertainty is preserved.

Disposition D

Systemically Propagating / Redesign Required

Evidence supports material systemic propagation or feedback that current governance cannot sufficiently contain within the examined boundary. Redesign may be supported as a governance conclusion.

Disposition U

Governability Unresolved

The record is not sufficient to determine whether the relevant system boundary is governable, distributed-but-governable, or requires redesign. This is a legitimate terminal outcome.

Systemic distribution does not equal ungovernability.
A legitimate terminal outcome may still be unresolved.
No Forced Closure

The absence of a further phase does not authorize manufactured certainty.

FVM is the terminal phase of the Obraval Governance Standard.

That creates a particular methodological responsibility.

There is no automatic deeper phase available to absorb uncertainty.

But the absence of another phase does not authorize FVM to manufacture an answer.

Where the record cannot responsibly distinguish among the available terminal positions, the unresolved state remains part of the governance record.

The absence of a further phase does not authorize the methodology to convert uncertainty into certainty.

Terminal does not mean closure at any cost.

It means the methodology has reached the edge of its authority.

Governance Options Classification

Governance options are classified, not prescribed.

FVM may classify governance options supported by the record, including:

  • no additional diagnostic action;
  • targeted evidence acquisition or monitoring;
  • bounded governance redesign;
  • cross-silo coordination or decision-right redesign;
  • control or monitoring architecture redesign;
  • specialist legal, audit, regulatory, or technical review outside FVM;
  • or authorized acceptance of residual exposure.

But FVM does not prescribe implementation steps.

It does not create legal strategy.

It does not direct financial action.

It does not become an operational remediation plan.

Governance conclusion is not implementation instruction.
Institutional Boundary

What FVM does not claim.

FVM is not:

  • a digital twin;
  • a simulation engine;
  • an audit opinion;
  • a legal opinion;
  • a regulatory finding;
  • or a prediction system.

It does not treat:

  • a matrix as proof of dynamic behavior;
  • cross-silo presence as systemicity;
  • distribution as propagation;
  • materiality as systemicity;
  • difficulty as irreversibility;
  • or matrix density as ungovernability.
Systemic language should be earned separately from the existence of the matrix.

The organization provides:

  • the operating context;
  • the independent challenge;
  • and the executive judgment.

The methodology provides:

  • the governability examination;
  • and the terminal disposition structure.

The organization retains responsibility for the judgment and any action that follows.

Methodology structures the examination. Evidence constrains the claims. Leadership retains the judgment.

Systemic Thinking

Systemic thinking without manufactured certainty.

The purpose of FVM is not to prove that a system is broken.

It is to determine what the evidence actually supports about relationships, propagation, attribution, reversibility, governance-response capacity, and governability.

A condition may be distributed without propagating.
A serious condition may remain locally governable.
A difficult condition may remain reversible.
A coherent chain may remain evidentially incomplete.
And a terminal examination may still end with: we do not yet know.

A map is not a model.

A relationship is not propagation.

Cross-silo presence is not systemicity.

Materiality is not systemicity.

Unknown is not failure.

Where the evidence cannot support terminal certainty, unresolved remains part of the governance record rather than being converted into a stronger conclusion.
